LifeCare Management services benefit our clients and their families in two major ways. First, they allow adults or elderly to remain safely at home and to maintain quality of life and care. Second, our services significantly lessen the burden on families of our clients. We can set up and manage care for your loved one so that you don’t have to. And when we say that we are affordable  – we mean it! We charge lower rates than larger agencies, while the independent caregivers whom we refer to our clients charge lower rates than the agency-employed ones. Thus, our clients save both ways – on care management and on care provider services. This affords our clients to retain and maintain essential and valuable services for as long as care is needed.

 

LifeCare Management Services Include:

 

  • Local and long distance care management for clients and their families

  • Comprehensive Assessment of adult or elder client’s status, needs, and ability to live at home safely and independently

  • Developing a customized Plan of Care with recommendations for the client

  • Implementing the care plan and managing care to meet the client’s needs

  • Communicating with doctors and other care providers, coordinating of services for the client

  • Arranging and accompanying client to medical and other appointments, and advocating for the client to assure best possible care

  • Home visits, phone consultations and ongoing care management support

  • Identifying and referring independent caregiver candidates to the client

  • Referring nursing and other agencies to the client

  • Facilitating the client's safe discharge from a hospital or rehab facility to home

  • Arranging medical equipment and assistive devices for the client

  • Assistance with medical billing, the Advanced Health Care Directive and other paperwork

  • Help navigating through the maze that is a health care system

  • Regular reports and recommendations to the client’s family

  • Researching and evaluating various facility care options and making recommendations to the client and their family

  • Facility referral and client placement if needed
